Greens Fork Animal Hospital Boarding Information

Pets may be dropped off or picked up during the following hours:

7:30 am to 5:00 pm      Monday through Friday
7:30 am to 11:00 am    Saturday

Allow 10 to 15 minutes to fill out boarding information and to have your pet checked in by one of our technicians.  To speed the admission process, you can visit our website and fill out the boarding information forms online.

No pets will be admitted or discharged on holidays or Sundays.  NO EXCEPTIONS.

Make reservations as early as possible especially during holiday season and through the summer months.  Four to six weeks should be adequate.
When you drop your pet off for boarding, please bring any medical problems to our attention which need to be checked by a doctor by making the appropriate notations on the front of this form. We may need to clarify the condition with you before you leave to better care for your pet.  There will be an additional charge for any examination requested while your pet is boarding.

Bring any medications your pet may be currently taking.  There will be an additional charge for the administration of medications while boarding.

On the front of the form, there is space to leave a name and phone number of a person to contact in case an emergency arises with your pet while boarding.

For the health and safety of your pet and others in the hospital, we require:

1.  Written proof of current vaccinations including Bordetella
2.  Treatment with one of the following topical flea medications within the 30 day period before boarding Frontline, Advantage, Revolution, or K9 Advantix.

Heartworm prevention is recommended in dogs and cats.

Pets will be fed Hill's Science Diet dry or canned food.  If your pet has specific diet requirements, please feel free to bring this diet from home.

We provide individual cages and runs.  Feel free to bring any special toys, treats, or favorite bedding for your pets to make them feel more comfortable during their stay here.

Playtime is available for your pet while boarding!

If you would like a little extra TLC for your pet, we offer the option of staff supervised playtime and exercise for your canine or feline companion while they stay with us.
Twice a day, our staff can spend 15 minute sessions with your dog or cat. For dogs, we take them into our enclosed grassy area to play ball, run around and/or just sit in the grass and be petted.  If the weather is bad, they will be taken to our indoor training facility on the lower level.  Cats will be taken to our downstairs apartment to chase toys, sit in the sun or just enjoy being petted.

The cost is $15.50 per day plus our regular boarding rate.
